Complaints to the NHS

Information is compiled from data collection forms K041(A) and K041(B).

All complaints which go into writing at any stage of the complaints procedure should be recorded on the K041 returns. The K041(A) return collects information about complaints relating to hospital and community health services (H&CHS), while the K041(B) collects information about complaints relating to family health services (FHS).
